Signers who wish to have their signature removed from a petition must submit a proper request to the County Clerk. The statement must include:
- The voter’s name
- The residential address of the voter, as shown in voter registration system
- A request for signature removal from a petition (identify the petition in your request)
- A written signature (digitally signed requests will not be accepted)
- A date for the request
To increase the likelihood of signature removal, please optionally include:
- A birthdate

There is not a specific form required and it does not have to be notarized.
